Approval of $50,000 in grant funding from an Anonymous Foundation
On August 28, 2018, the Lake County Health Department and Community Health Center (LCHD/CHC) received $50,000 in funding to support operational costs at the Round Lake Area Student Health and Wellness Center. Per the conditions of the grant award, the grant is to be recognized as "anonymous" and the name of the foundation is not to be used in any publication. This includes Governing Council documents (agendas, minutes, etc.) which are publicly available. The grant supports operational costs at the Round Lake Area Student Health and Wellness Center including salary/fringe dollars for the provider and supply/operational costs. The grant period for this award is September 1, 2018 - August 31, 2019.

Approval of $119,771 in quality improvement funding from the Health Resources and Services Administration
On August 2, 2018, the Health Resources and Services Administration (HRSA) awarded LCHD/CHC a total of $119,771 in quality improvement funding. The amount received was based on: 1) LCHD/CHC maintaining Primary Care Medical Home (PCMH) recognition; 2) LCHD/CHC using its Electronic Health Record (EHR) to report population-level data for quality indicators; 3) LCHD/CHC utilizing health information technology systems to increase access to care and advance quality of care; and 4) LCHD/CHC being recognized as a "clinical quality improver" by demonstrating improvements in quality measures as reported through the Uniform Data System report. These one-time funds were added to LCHD/CHC's section 330 grant. The grant period is August 1, 2018 - July 31, 2019.

As noted in the attached budget, quality improvement funds will be used to support quality-related software, training and supplies. Specifically, funds will support acquisition of the following items:
* Staff training.
* Supplies for LCHD/CHC's simulation lab.
