Approval of $50,000 in grant funding from an Anonymous Foundation

On August 28, 2018, the Lake County Health Department and Community Health Center (LCHD/CHC) received $50,000 in funding to support operational costs at the Round Lake Area Student Health and Wellness Center.  Per the conditions of the grant award, the grant is to be recognized as “anonymous” and the name of the foundation is not to be used in any publication.  This includes Governing Council documents (agendas, minutes, etc.) which are publicly available.  The grant supports operational costs at the Round Lake Area Student Health and Wellness Center including salary/fringe dollars for the provider and supply/operational costs.  The grant period for this award is September 1, 2018 - August 31, 2019. 

 

Approval of $119,771 in quality improvement funding from the Health Resources and Services Administration

On August 2, 2018, the Health Resources and Services Administration (HRSA) awarded LCHD/CHC a total of $119,771 in quality improvement funding.  The amount received was based on: 1) LCHD/CHC maintaining Primary Care Medical Home (PCMH) recognition; 2) LCHD/CHC using its Electronic Health Record (EHR) to report population-level data for quality indicators; 3) LCHD/CHC utilizing health information technology systems to increase access to care and advance quality of care; and 4) LCHD/CHC being recognized as a “clinical quality improver” by demonstrating improvements in quality measures as reported through the Uniform Data System report.  These one-time funds were added to LCHD/CHC’s section 330 grant.  The grant period is August 1, 2018 - July 31, 2019.

 

As noted in the attached budget, quality improvement funds will be used to support quality-related software, training and supplies.  Specifically, funds will support acquisition of the following items:

                     Staff training.

                     Supplies for LCHD/CHC’s simulation lab.

                     Additional licenses for LCHD/CHC’s data visualization software.

                     Template development for LCHD/CHC’s Electronic Health Record.

 

Approval of $406,176 in grant funding from the Health Resources and Services Administration and related program income  

HRSA has provided LCHD/CHC with $344,500 in funding through the Expanding Access to Quality Substance Use Disorder and Mental Health Services (SUD-MH) funding opportunity.  The initial budget period is September 1, 2018 - August 31, 2019 with an opportunity for carryover into the next grant year.  Of the $344,500 in funding, $175,000 is one-time funding and $169,500 is anticipated annual funding.  Anticipated annual funding is included in the regular 330 grant award.  Grant funds will be used to support and expand medication-assisted treatment (MAT) services at LCHD locations.  Two substance abuse counselors, a medical assistant and a clerk will be added to support the MAT program.  One-time funds will be used for facility improvements at the 3004 Grand Avenue location, supplies for new staff and software development with the agency’s host provider (Emed apps).

 

In addition to the grant award, LCHD/CHC projects program income from new visits delivered by SUD-MH funded staff to be $61,676.  This includes $50,530 from Medicaid, $1,588 from Medicare, $9,003 from private insurance companies and $555 in patient fees from uninsured patients.  This, together with the grant award, totals $406,176 for the total program budget.  Program income will be used to fund supportive positions including half of the medical assistant position and 100% of the clerk position. 

 

Approval of $125,000 in grant funding from the Substance Abuse and Mental Health Services Administration

The Substance Abuse and Mental Health Services Administration (SAMHSA) has awarded the LCHD/CHC a three-year grant in the amount of $375,000.  The first budget period runs September 30, 2018 - September 29, 2019 and includes $125,000 in grant funding.  Subsequent budget periods are also funded at $125,000 per year. 


The project will fund a mental health first aid train-the-trainer program for adults in Lake County.  Over three years, LCHD/CHC will identify 90 individuals to serve as mental health first aid trainers.  These individuals in turn will train 2,250 community members in mental health first aid.  Grant funds will be used to cover salary and supply costs for program administration as well as travel and contract costs associated with training. Some tax offset is included for existing staff. 

 

Approval of $500,000 in grant funding from the US Department of Justice Office of Justice Programs Bureau of Justice Assistance

The US Department of Justice Office of Justice Programs Bureau of Justice Assistance (BJA) has awarded LCHD/CHC a two-year grant in the amount of $500,000.  The grant has one budget period which runs from October 1, 2018 - September 30, 2020.


The funding will support the A Way Out program.  A Way Out, launched in 2016, helps connect individuals who are addicted to opioids or other drugs to substance abuse treatment.  Participants can walk in to one of 13 participating police departments 24 hours a day, 7 days a week without fear of prosecution.  LCHD/CHC staff will provide screening and find appropriate treatment for the person seeking help. 

 

Through the grant, LCHD/CHC will hire three full time staff to operate and grow the program.  Grant funds will also be used to attend required conferences, purchase outreach supplies, evaluate the program and provide patient assistance funding to reduce out-of-pocket costs for participants.  As it is a two-year grant, $247,621 will be appropriated initially in the LCFY19 budget and the balance will be included in the LCFY20 budget.

 

Approval of $147,093 in additional funding from the Illinois Department of Public Health for the Illinois Breast and Cervical Cancer Program

The Illinois Department of Public Health (IDPH) has provided LCHD/CHC with $147,093 in additional funding for the Illinois Breast and Cervical Cancer Program (IBCCP).  These additional funds were awarded in two grant contract years:

 

                     $88,155 in additional funding was awarded for the budget period that ran from July 1, 2017 - September 30, 2018;

                     $58,938 in additional funding was awarded for the budget period that runs from October 1, 2018 - June 30, 2019.

 

LCHD/CHC has been the lead agency in Lake County for the IBCCP program for over 10 years.  The program provides mammograms and cervical cancer screenings to uninsured women in Lake County.  The additional funding was awarded as the state IBCCP program extended its state fiscal year 2018 contract and then assigned an award amount for its state fiscal year 2019 contract.  This appropriation will align the LCHD/CHC county budget with the awarded amounts for the two grant periods.  Additional funding will be used for new salary/benefit costs for IBCCP including the creation of a new Public Health Assistant position and overtime for existing staff.  The remaining dollars will be used for the clinical costs of mammograms. 

 

Suggested Motion

To approve the additional grant funding of $50,000 from an anonymous foundation, $119,771 from HRSA, $406,176 from HRSA and related program income, $125,000 from the SAMHSA, $500,000 from BJA and $147,093 in additional funding from the IDPH.
