The Lake County Health Department and Community Health Center (LCHD/CHC) desires to purchase twelve (12) vehicles that were budgeted for fiscal year 2017 and one (1) vehicle, new to the fleet, to be purchased with SAMHSA Primary and Behavioral Health Care Integration (PBHCI) grant dollars for a total of thirteen (13) vehicles, as follows:

4 each: Vehicle #1 - 2017 Ford Focus S Sedan - (3) for Prevention Administration located at the Central Permit Facility and (1) for Prevention Tuberculosis Program

3 each: Vehicle #2 - 2017 Ford Escape S - (2) for Prevention Administration located at the Central Permit Facility and (1) for Prevention Ecological Services program.

1 each: Vehicle #3 - 2017 Ford Super Duty Pick-Up F250 for Business Operations Facilities program.

2 each: Vehicle #4 - 2017 Ford Transit Wagon T150, 8-passenger - (1) for Clinical Operations, Child and Adolescent Behavioral Services (CABS) program and (1) for Clinical Operations Community Support Services (CSS) and Case Management (CM) programs.

1 each: Vehicle #5 - 2017 Ford Transit Wagon T350, 15-passenger, for Clinical Operations, Mental Health Outpatient program. New to fleet, to be purchased with grant funding.

1 each: Vehicle #6 - 2017 Ford Transit Van T150 for Prevention Animal Care and Control program.

1 each: Vehicle #7 - 2017 Ford Transit Van T150 for Business Operations Facilities program.

? A legal notice was published in the Lake County News Sun soliciting sealed Bids for all vehicles listed above.
? The bid was posted to the Lake County external Purchasing website and subsequently, email notifications were sent to registered suppliers.
? Sealed Bids were received from three (3) suppliers and opened publicly.
? The total cost to purchase all thirteen (13) vehicles is $292,031.00. This total is below the amount budgeted for vehicles in FY2017.
