* There is a need to enter into a contract for health risk assessments (HRA) services for Lake County due to a new stipulation, effective January 2016, requiring all new employees and all employees enrolled in the consumer driven health plan complete a HRA.?

* Based on the criteria set forth in the Request for Proposals, the evaluation committee selected Catapult Health, Dallas, Texas, as the most favorable proposal for Lake County.
* The contract authorizes a two year contract, with three additional one year renewals, that will cost the County an estimated annual amount of $130,000.

RESOLUTION
WHEREAS, in accordance with Section 5-102 of the Lake County Purchasing
Ordinance, proposals were called for by publication for health risk assessments services for Lake County; and
WHEREAS, subsequent to said publication, requests for proposals were extended to 20 vendors; and
WHEREAS, sealed proposals were received from three vendors and opened in the County Building, Waukegan, Illinois, on March 17, 2016 at 2:00 p.m.; and
WHEREAS, an evaluation committee, consisting of representatives from the Human Resources and Finance and Administrative Services, held interviews with the top two firms; and
WHEREAS, based on the criteria set forth in the Request for Proposal, the evaluation committee selected Catapult Health BMI, Dallas, Texas, as the most favorable proposal for Lake County.
