• There is a need to enter into a contract for health risk assessments (HRA) services for Lake County due to a new stipulation, effective January 2016, requiring all new employees and all employees enrolled in the consumer driven health plan complete a HRA.
• A Request for Proposal was extended to 20 vendors, sealed proposals were received from three vendors, and interviews were held with two firms.
• Based on the criteria set forth in the Request for Proposals, the evaluation committee selected Catapult Health, Dallas, Texas, as the most favorable proposal for Lake County.
• The contract authorizes a two year contract, with three additional one year renewals, that will cost the County an estimated annual amount of $130,000.

RESOLUTION
WHEREAS, in accordance with Section 5-102 of the Lake County Purchasing
Ordinance, proposals were called for by publication for health risk assessments services for Lake County; and
WHEREAS, subsequent to said publication, requests for proposals were extended to 20 vendors; and
WHEREAS, sealed proposals were received from three vendors and opened in the County Building, Waukegan, Illinois, on March 17, 2016 at 2:00 p.m.; and
WHEREAS, an evaluation committee, consisting of representatives from the Human Resources and Finance and Administrative Services, held interviews with the top two firms; and
WHEREAS, based on the criteria set forth in the Request for Proposal, the evaluation committee selected Catapult Health BMI, Dallas, Texas, as the most favorable proposal for Lake County.
NOW, THEREFORE, BE IT RESOLVED, by this County Board of Lake County, Illinois, that that the Purchasing Agent is authorized to enter into a contract with Catapult Health, Dallas, Texas, for health risk assessments services for Lake County for a two year period, plus renewals, in the estimated annual amount of $130,000 charged to 510-1430010-71150.
