Presented is one (1) provider for credentialing and privileging. The Interim Medical Director has verified the privileges requested are appropriate for the provider’s training and experience. The Interim Medical Director, the Board of Health Credentialing Committee, and the Governing Council Personnel Committee has studied the application, raised questions where necessary, and based upon satisfactory response, recommends approval of the request for privileges.
