Presented are two (2) providers for appointment and one (1) provider for reappointment. The Medical Director has verified the privileges requested are appropriate for the providers’ training and experience. The Medical Director and the Board of Health Credentialing Committee has studied the applications, raised questions where necessary, and recommends approval.
