Ratification of Request for Proposals #22097 - Midlakes and Zion Health Center Roof Rehabilitation - Nordstrom

Lake County Health Department and Community Health Center (LCHD/CHC) has a need to enter into an agreement with a contractor for roof rehabilitation at the Midlakes health center and the Zion health center. Funding for these rehabilitations is from the HRSA ARPA grant.
A legal notice was published in the News Sun on September 22, 2022, and the bid document was posted to the Lake County Purchasing website.
An invitation to bid was issued and nineteen vendors registered. Sealed bids were received from two contractors on October 6, 2022, in amounts ranging from $380,000 to $488,000.
Waukegan Roofing Co., Inc., Mundelein, Illinois, was determined to be the lowest responsive and responsible bidder who best meets the needs of LCHD/CHC in the amount of $380,000. LCHD/CHC has positive prior experiences with Waukegan Roofing.
Suggested Motion
To ratify and approve the award and authorize the Executive Director or designee to execute the necessary documents for a contract with Waukegan Roofing Co., Inc., Mundelein, Illinois, in the amount of $380,000 for necessary roof rehabilitation at the Midlakes health center and the Zion health center.
