Presented are seven (7) providers for reappointment. The proper verifications have been performed and there were no identified issues. The Medical Director, the Board of Health Credentialing Committee, and the Governing Council Personnel Committee, has studied the applications, raised questions where necessary, and based upon satisfactory responses, recommends approval of the requests for reappointment.
