The UDS Clinical Quality Indicator dashboard is attached. The green bars indicate health center performance for 2021. The blue bars indicate current health center performance. Current performance is similar to 2021 performance across most metrics with slight variation in some measures. In 2022, we are focusing on improving diabetes care, childhood immunization rates, depression remission, childhood weight assessment, and colorectal cancer screening.

 

We are working on two Quality Improvement projects at the Midlakes Health Center. Toni Steres, Director of Advanced Practice, is leading a Plan-Do-Study-Act (PDSA) to increase the percentage of pediatric patients that receive counseling on physical activity and nutrition during their medical visit. The overall goal is to increase the percentage of pediatric patients with weight assessment and counseling from 58.1% to 80% by December 31, 2022. The Quality Improvement team is leading a PDSA with Carlie Terchin, APRN, to increase the percentage of patients who complete an at-home colorectal cancer screening test. The overall goal is to increase the percentage of patients with a colorectal cancer screening from 32% to 50% by December 31, 2022. Both PDSAs are currently in the planning and data collection phase.
