RESOLUTION
WHEREAS, in accordance with Section 33.050 of the Lake County Purchasing Ordinance, proposals were called for by publication for occupational health services for Lake County; and
WHEREAS, subsequent to said publication, request for proposals were extended to 11 vendors; and
WHEREAS, sealed proposals were received from three vendors, on June 28, 2022, at 11:00 a.m.; and
WHEREAS, based on the criteria set forth in the Request for Proposals, an ad hoc committee selected Advocate Aurora Health, Libertyville, IL, to be in the best interest of Lake County.
NOW, THEREFORE, BE IT RESOLVED, by this County Board of Lake County, Illinois, that the Purchasing Agent be authorized to enter into the contract with Advocate Aurora Health to complete vaccination, Personal Protection Equipment (PPE) fit testing, and pre-employment physicals for County employees in an estimated annual amount of $278,740 out of account number 101-1400010-71420-000-000-000-00000.
