Title

Request for Proposals - E-Consult Services for LCHD/CHC #22025 - Nordstrom

Body

Lake County Health Department and Community Health Center (LCHD/CHC) is seeking a qualified firm to provide E-Consultant services. The purpose of this RFP is to establish a contract to provide e-consult services for enhancing LCHD/CHC patient care. The e-consult service is based on written communication from the provider to a specialist on a timely basis. The e-consult service does not directly see or evaluate the patient. ARPA funds have been allocated to provide specialty services via e-consults for unfunded patients.

A legal notice was published in The News Sun on March 3, 2022, soliciting Requests for Proposals (RFP 22025), and the RFP was posted to the LCHD/CHC website. Three suppliers submitted sealed proposals which were opened in the Materials Management office on March 24, 2022.

An evaluation committee comprised of LCHD/CHC staff reviewed, short listed, and interviewed two proposers and determined that RubiconMD Inc, of New York, New York, best meets the needs of LCHD/CHC.

Based on the RubiconMD proposal, the contract dollar amount totals $56,200 in year one and $51,204 for year two, including options for three additional one-year renewals subject to acceptable performance by the contractor. Prices will remain firm/fixed for years one and two. The supplier must submit a written request 60 days in advance of the annual contract period for any future price increases after the initial two-year contract period.

Suggested Motion

To approve the award for RFP #22025 for E-Consultant services for LCHD/CHC to RubiconMD of New York, NY in the amount of $56,200 in the first year and $51,204 for year two of the contract and authorize the Executive Director or designee to execute the necessary negotiated documents.
