• Culvert replacements on Gilmer Road, Monaville Road, and Old McHenry Road are currently under contract with Lake County Grading, Libertyville, Illinois, in the amount of $318,536.25.
• The County Board, at its July 13, 2021 meeting, approved an appropriation of $385,000 of County Bridge Tax funds for this project.
• Change orders need to be approved by the standing committees for cumulative net increases over 10 percent.
• The sum of all contract change orders to date is $78,471.58, which represents a 24.64 percent increase over the original awarded contract amount.
• This project is included in the Transportation Improvement Program, and designated as Section 18-00999-58-DR.

R E S O L U T I O N
WHEREAS, Gilmer Road, County Highway 26, Monaville Road, County Highway 55, and Old McHenry Road, County Highway 32, are designated routes on the County highway system; and
WHEREAS, Lake County believes it would be beneficial to the safety and welfare of the motoring public if Gilmer Road, Monaville Road, and Old McHenry Road could be improved by the replacement of culverts; and
WHEREAS, the County Board, at its meeting of July 13, 2021, approved an appropriation of $385,000 of County Bridge Tax funds for the replacement of various culverts on Gilmer Road, Monaville Road, and Old McHenry Road; and
WHEREAS, this construction project is under contract with Lake County Grading, Libertyville, Illinois, in the amount of $318,536.25; and
WHEREAS, Change Order Number Four, in the amount of $49,000, consisted of the earned incentive for the early completion of Monaville Road; and
WHEREAS, Change Order Number Three, in the amount of $1,148.20, consisted of balancing quantities to final measurements up to the limit that the County Engineer could approve; and
WHEREAS, Change Order Number Two, in the amount of $6,179.59, consisted of extra work to cut down a masonry wall; and
WHEREAS, Change Order Number One, in the amount of $22,143.79, consisted of the earned incentive for the early completion of Gilmer Road as well as balancing quantities to final measurements; and
WHEREAS, a supplemental appropriation of funds is necessary for the additional scope of work; and
WHEREAS, the Lake County Purchasing Ordinance requires the Public Works, Planning and Transportation Committee and the Finance and Administrative Committee to approve change orders with cumulative totals above 10 percent of the original contract.
NOW, THEREFORE BE IT RESOLVED by this Lake County Board, that the County Engineer of Lake County is authorized and directed to execute on behalf of Lake County, Change Order Number Four in the amount of $49,000 for the replacements of various culverts on Gilmer Road, Monaville Road, and Old McHenry Road.
BE IT FURTHER RESOLVED that there is appropriated a supplemental amount of $12,100 of County Bridge Tax funds for replacement of various culverts on Gilmer Road, Monaville Road, and Old McHenry Road, and designated as Section 18-00999-58-DR.
