Presented is one provider for appointment and one provider for reappointment. These providers were approved by the Board of Health Credentialing Committee on Wednesday, November 17, 2021, and by the Board of Health Executive Committee on Monday, November 22, 2021. The Medical Director has verified the privileges requested are appropriate for the provider’s training and experience. The Medical Director, the Board of Health Credentialing Committee, and the Board of Health Executive Committee has studied the applications, raised questions where necessary, and recommends approval.
