RESOLUTION
WHEREAS, in accordance with Section 5-102 of the Lake County Purchasing Ordinance, proposals were called for by publication for Dental Insurance for Lake County; and
WHEREAS, subsequent to said publication, requests for proposals were extended to 26 vendors and received from five; and
WHEREAS, an evaluation committee made up representatives from the Human Resources Department reviewed all submitted documents using an individualized score card; and
WHEREAS, based on the criteria set forth in the Request for Proposal, the Evaluation Committee comprised of representatives from Human Resources selected Delta Dental, Naperville as the most favorable proposal for Lake County.
NOW, THEREFORE, BE IT RESOLVED, by this County Board of Lake County, Illinois, that the Purchasing Agent is authorized to enter into a contract with Delta Dental, Naperville, IL for Dental Insurance Services for Lake County for a one (1) year period plus renewals in the estimated annual amount of $2,283,848.64 charged to 510-1430010-74030.
