Presented are three (3) providers for credentialing. The Medical Director has verified the privileges requested are appropriate for each provider’s training and experience.
The Medical Director has studied the applications, raised questions where necessary, and based upon satisfactory responses from the applicants, recommends approval of each request for privileges.

• Kelly Ratz, Psychiatric-Mental Health Nurse Practitioner (PMHNP)
• Isabel Rojas, Doctor of Podiatric Medicine (DPM)
