Provider Collaboration

                     School-based Health Center (SBHC) APRN Shannon Trocki has transferred to an opening in Women’s Health.

                     Zion Health Center APRN Franceliz Recinto has transferred to the SBHC.

                     Recruitment of an APRN to fill the position at the Zion Health Center clinic is ongoing.

 

Operations

                     The move of providers from the North Chicago Health Center (NCHC) to the Belvidere Medical Building (BMB) is complete.

                     We await approval by the Lake County Board of the ARPA FQHC grant. In preparation for that approval, RFPs and proposal requests are being done for e-consult services, credentialing software, and a value-based consulting service.

                     Focus of patients: Attached is a report of patients’ diagnoses and, as noted, we serve a large number of patients with behavioral health diagnoses, obesity, and hypertension.

 

External Relations

                     The LCHD/CHC Medical Director awaits the granting of privileges at Advocate Condell.

                     Privileges at local hospitals enables better tracking and coordination of care of LCHD/CHC patients seen in the ED or hospitalized at those facilities.

 

Regulatory

                     Continuous regulatory readiness for The Joint Commission (TJC) and HRSA has begun following completion of both surveys this year.

 

Psychiatry/MAT

Provider Collaboration

                     No updates at this time.

 

Clinic Operations

                     No significant FQHC-related changes in clinical operations. Most services continue to be provided using telephone or video encounters, though face-to-face appointments are available daily.

                     The Behavioral Health Medical Director position is posted.

                     The use of telehealth vendors to add more psychiatry capacity is being researched.

 

External Relations

Regulatory

                     No updates at this time.

 

Dental

Provider Collaboration

                     Recruitment of a dental hygienist for the Zion Health Center location is ongoing.

                     The dental assistant position at the Midlakes Health Center remains open.

                     There are no dental provider positions open at this time.

 

Clinic Operations

                     Annual sterilization observation audits for Dental Assistant and Women’s Health staff are being conducted.

                     All dental staff participated in an online Infection Control and Instrument Training update as part of their annual update training.

                     A new water filtration system has been installed and is now operational at BMB. Our goal is to have this system implemented at all of our dental practice sites in the future.

 

External Relations

                     Current key performance indicators (KPI) for the department are over 90% compliant and planning for updating of current measures is ongoing. Current KPIs include Caries Risk Assessment Completion and Fluoride Applications in children under 18 years of age. As a new KPI measure, we plan to include medication reconciliation beginning December 2021.

                     The Dental HRSA UDS measure of application of dental sealants in moderate to high-risk children ages 6-9 years on their permanent 1st molar continues to be monitored. This measure’s compliance was negatively impacted by COVID-19. Currently, the measure is 81% compliant but this has improved over the past several months.
