On June 28, 2021, Illinois Children’s Healthcare Foundation (ILCHF) and Delta Dental of Illinois Foundation (DDILF) announced they are accepting applications for the FY2022-2023 funding opportunity. LCHD/CHC has been given the opportunity to apply for $200,000. The American Dental Association (ADA) identified that many persons in underserved areas suffer from untreated disease, and many more are at risk of disease, often because they don’t have access to preventative dental health care and education. This grant will address this deficiency. Grant funding would be used provide oral health education, connection to dental services and other needed social and economic resources to the underserved pediatric patients in Lake County, most affected by the COVID-19 pandemic. We plan to use the ADA’s model of community outreach, using trained community health workers and case managers supervised by a dental hygienist. We plan to measure our success by: tracking the number of pediatric patients who obtain dental care, and the number of pediatric patients connected to other needed social and economic resources, and the overall dental health in our community.
The grant period is January 1, 2022 - December 31, 2023. The team tasked with developing the program proposal is comprised of Kim Burke, Omar Cockey, DDS, Leslie Zun, MD, Diane Pelli, and Pam Riley. The team completed the program proposal in response to Notice of Funding Opportunity by ILCHF and DDILF.
