Presented are six (6) providers for credentialing. The Medical Director has verified the privileges requested are appropriate for each provider’s training and experience.
The Medical Director has studied each application, raised questions where necessary, and based upon satisfactory responses from the applicant, recommends approval of the request for privileges.
