Scope of Project Change - Additional Services - Podiatry - Dr. Zun

As a federally qualified health center (FQHC), the Lake County Health Department and Community Health Center (LCHD/CHC) is required to maintain a scope of project with the Health Resources and Services Administration (HRSA). The scope defines the approved service sites, services, providers, service areas and target populations served with section 330 grant funds. LCHD/CHC is responsible for updating this scope of project as organizational services change.
LCHD/CHC is finalizing a referral agreement with Rosalind Franklin University of Medicine and Science (RFUMS) to provide podiatry services within LCHD/CHC FQHC sites. For dates of service January 1, 2021 thru June 30, 2021, LCHD/CHC providers generated 382 orders for external podiatry services. Podiatry services are required for many of our patients with diabetes. Having podiatry services at our FQHC site(s), will expand access for our patients to these specialty care services.
The scope change, podiatry as an additional service, needs to be approved by the Governing Council. If Governing Council approval is obtained, LCHD/CHC will submit a formal addition in scope request to HRSA.
For the purposes of the scope:
• Column I services are provided directly by LCHD/CHC;
• Column II services are provided contractually with LCHD/CHC paying for the services;
• Column III services are provided through a contractual, referral relationship where LCHD/CHC does not pay for the service.
Podiatry services provided by RFUMS would be added to Column III services.
