As a federally qualified health center (FQHC), the Lake County Health Department and Community Health Center (LCHD/CHC) is required to maintain a scope of project with the Health Resources and Services Administration (HRSA). The scope defines the approved service sites, services, providers, service areas and target populations served with section 330 grant funds. LCHD/CHC is responsible for updating this scope of project as organizational services change.

LCHD/CHC is going to start a $1,470,131 renovation of the North Chicago Health Center (NCHC) for general interior space changes to improve the quality of care, promote operational efficiencies, and install a building generator. The general interior renovations include improvements to the reception area, dental operatories/sterilization areas, medical team rooms, and laboratory spaces as the most significant areas of construction. These improvements are funded through the Community Health Center Construction Act Grant Program via the Illinois Capital Development Board. This is the third of three projects approved in our grant application which also included work previously completed at the Belvidere Medical Building and currently in process at the Zion Health Center.

In order for timely completion of the renovation along with safety and infection control mitigation, LCHD/CHC Leadership is planning to close the NCHC for 6-8 months during the renovation. NCHC medical and dental providers, their teams, behavioral health, and WIC services will be relocated to either the Belvidere Medical Building or Grand Avenue Health Center to help assure ongoing, comprehensive continuity of care for patients.

The service site scope change, closure of NCHC during the renovation, needs to be approved by the Governing Council. If Governing Council approval is obtained, LCHD/CHC will submit a formal service site scope change request to HRSA.
