Change in FQHC Hours of Operation - Dr. Zun

As defined in the Governing Council bylaws, one of the responsibilities assigned to the Council is the approval of hours of services delivered at Lake County Health Department and Community Health Center (LCHD/CHC) Federally Qualified Health Center (FQHC) locations. LCHD/CHC is bringing the attached revised hours of operation for the FQHC sites for review and approval.

To approve the hours of operation for the LCHD/CHC FQHC sites as presented.
