Lake County Health Department Diabetes Care Initiative - $250,000 - Grant Approval - Pfister

On July 19, 2021, Aetna Better Health of Illinois Inc. (ABHIL) awarded Lake County Health Department and Community Health Center (LCHD/CHC) a total of $250,000 for the period July 1, 2021 - June 30, 2023. These funds will be used to improve diabetes care for LCHD/CHC patients which will include 95% of ABHIL members with uncontrolled diabetes. LCHD/CHC will hire staff to pilot a care management model and purchase necessary supplies and equipment to provide patients with the support and supplies to improve their health. LCHD/CHC use of these funds are aligned to support salaries, fringe benefits, A1c point of care testing machines, and glucose monitoring kits.
Suggested Motion
To approve the Aetna Better Health of Illinois Inc. (ABHIL) funding in the amount of $250,000 for the diabetes care initiative.
