Scope Change - Clarification of Services Provided - Burke

As a federally qualified health center (FQHC), the Lake County Health Department and Community Health Center (LCHD) is required to maintain a scope of project with the Health Resources and Services Administration (HRSA). The scope defines the approved service sites, services, providers, service areas and target populations that are served with section 330 grant funds. LCHD/CHC is responsible for updating this scope of project as organizational services change.

 

LCHD/CHC’s scope was reviewed in preparation for the August 2021 HRSA site visit. As a result, one adjustment in scope needs to be made as proposed on the attached. It does not represent a change in services but clarifies the scope so that it accurately reflects the services currently provided by LCHD/CHC.

 

This scope adjustment needs to be approved by the Governing Council. If Governing Council approval is obtained, LCHD/CHC will submit a formal adjustment in scope request to HRSA. The full process generally takes 1-2 months from Governing Council approval to final HRSA approval.
