FY2021 Health Resources and Services Administration (HRSA) American Rescue Plan (ARP) Health Center Construction and Capital Improvements (C8E) - $927,053 - Grant Application Approval - Nordstrom

On April 27, 2021, HRSA announced they are accepting applications for the FY2021 ARP C8E funding opportunity. LCHD/CHC has been given the opportunity to apply for $927,053. The purpose of this one-time funding opportunity is to support construction, expansion, alteration, renovation, and other capital improvements to modify, enhance, and expand health care infrastructure. HRSA determined the eligible award amount based on the following formula: $500,000 base plus $11.00 per patient reported in the 2019 Uniform Data System (UDS).
LCHD/CHC will apply for and use these funds for the following four potential project locations.
1) Renovations/Alterations to the Belvidere Medical Building including:
• exterior elastomeric painting
• NW exterior window removal and replacement
• fire alarm removal and replacement
• remodel spaces to accommodate additional and updated exam rooms, provide an additional team room, and relocate the laboratory
• update and add to HVAC system
2) Renovations/Alterations to the Midlakes Health Center including:
• exterior window removal and replacement
• flat roof repair, removal, and replacement of the main standing seam metal roofing
3) Renovations/Alterations to the Zion Health Center including:
• removal and replacement of the shingled/flat roof
4) Renovations/Alterations to the North Chicago Health Center including:
• removal and replacement of the exterior windows
The grant period is for three-years, September 1, 2021, through August 31, 2024. The team tasked with developing the program proposal is comprised of Kim Burke, Jerry Nordstrom, Bruce Robbins, Dr. Les Zun, Diane Pelli, and Pam Riley. The team is in the process of developing the program proposal in response to HRSA’s Notice of Funding Opportunity. The team requests Governing Council approval to move forward in applying for the HRSA Health Center Construction & Capital Improvements funding.

To approve the grant application for funding of up to $927,053 from the Health Resources and Services Administration.
