Physical Health

Provider Collaboration

                     Discussions are under way to offer a family medicine physician the Regional Medical Director position.

                     With the addition of the three new Pediatricians and the new Regional Medical Director, provider positions will be fully staffed.

 

Operations

                     Providers are adjusting to renovations taking place at our Belvidere Health Center.

                     Patient Focus: attached is a graph of the demographics of the patients we served in 2021; this graph includes age, sex, ethnicity, and race and the data demonstrates:

o                     The majority of our patient population identified as female

o                     The majority of our patient population is comprised of children and middle-aged persons

o                     Half of our patient population identified as Hispanic

 

External Relations

                     In order to improve continuity of patients who are admitted to the hospital, I have applied for privileges at Vista Medical Center - East, Northwestern Medicine Lake Forest Hospital, and Advocate Condell Medical Center. This partnership will enable the agency to view patient medical records, contact the admitting doctor, and speak with consultants.

                     Discussions are ongoing with the area hospitals to expand specialty referral options.

 

Regulatory

                     The Joint Commission (TJC) completed their review of the FQHC and we did exceptionally well with only five minor findings.

                     The HRSA site visit is confirmed for August 3 - 5, 2021 and preparations are ongoing.

 

Psychiatry - MAT

Provider Collaboration

                     No updates.

 

Clinic Operations

                     No significant FQHC-related changes in clinical operations. Most services continue to be provided using telephone or video encounters.

                     The Behavioral Health Medical Director position is posted.

                     Anatoliy Pyslar, MD, has joined the agency as a full time Psychiatrist; he will be treating patients at the Addictions Treatment Program, Crisis Care Program, Substance Abuse Program, and Grand Avenue Health Center.

 

External Relations

                     Nothing new to report.

 

Regulatory

                     No updates.

 

Dental Health

Provider Collaboration

                     Recruitment for the following open positions is ongoing:

o                     Full-time dentist at North Shore Health Center; thus far, eight resumes have been received.

o                     Full-time dental assistant at Midlakes Health Center (MLC); thus far, only three resumes have been received.

o                     Temporary dental assistant at MLC; very few candidates have applied.

o                     Student volunteer at MLC; Leadership has only one applicant anticipated to join the agency for a 100-hour rotation in June 2021. This individual has an interest in going to dental school.

                     No updates at this time.

 

External Relations

                     Nothing to report.

 

Regulatory

                     TJC review in April had no findings in infection control related to dental. Staff did a wonderful job when interviewed by TJC and congratulations are in order.

                     Dental Leadership and Infection Control Specialist have invested many hours in providing on-site consultations related to infection control and the sterilization processes. The goal is to maintain sustainability in the progress made. Moving forward, staff will continue to participate in site audits and trainings.
