Ratification of Pandemic Health Navigator Program Grant, $2,333,333 - McMillan-Wilhoit

On March 24, 2021, the Illinois Primary Health Care Association (IPHCA), an Illinois Not-for-Profit Corporation, awarded the Lake County Health Department and Community Health Center (LCHD/CHC) $2,333,333 for Pandemic Health Navigators (PHNs) to assist in the prevention of transmission of COVID-19 through education and outreach to vulnerable communities, coordination of resources for those infected or exposed, and provision of contact tracing support to the Illinois Contact Tracing Collaborative for the period February 1, 2021 through December 31, 2021.
LCHD/CHC use of these funds is aligned to support salaries, fringe benefits, computer, operational and medical supplies, computer equipment, contractual services, telecommunications, education, and training.
The application and funding for this grant was approved by the Governing Council Executive Committee on April 1, 2021.

To ratify the awarded funding related to expenses associated with the Pandemic Health Navigator Program Grant from the Illinois Primary Health Care Association in the amount of $2,333,333.
