Lake County Health Department and Community Health Center (LCHD/CHC) received new Schreiber Family Foundation grant funding in the amount of $88,600 for the period December 1, 2020 to November 30, 2021. The amount requested to be appropriated in LCFY21 is $88,600. This program is for Lake County children and families in need of home visit and supportive services. This will fund a new Community Systems Development Specialist, telecommunications, and mileage.
