Staff Summary
* Lake County Facilities identified a need to enter into a professional services agreement to provide architectural and engineering services for CARES Act Projects in August 2020 in accordance with the Local Government Professional Services Selection Act (50 ILCS 510/1 et seq) and a prior satisfactory relationship:
* Kluber Architects and Engineers was contracted to provide the required services for three CARES Act projects for the Health Department:
o Midlakes, North Shore and North Chicago heating, ventilating, and air conditioning (HVAC) upgrades;
o 3002 Grand Avenue Roof Top Unit HVAC replacement; and
o Belvidere Health Center Women's Health HVAC Improvements.
* The initial contract in the amount of $41,085 was for the construction document phase which included developing the scope of the project, providing specifications, developing technical drawings, providing front end bidding specifications, bid solicitation documentation, and contract provisions, and reviewing final documentation as authorized for this phase of project.
* Facilities has identified a need for Contract Modification Number One to increase the scope of work of the projects to include a construction administration phase in the amount of $49,670. This phase includes the review of contractor shop drawings, assistance in submission of plans, specifications, communications, and building permits, the review of data submittals, the review and monitor construction administration progress, and the review and confirmation of substantial completion timeline, final completion of work, and project hand-off after completion to Lake County.
