We are recommending the appointment and reappointment of the individuals noted in the suggested motion. For the individual recommended for appointment, we have performed the following verifications and there are no identified issues:
• Physical exam and drug screen
• Criminal background check
• Sex Offender registry check (CABS program providers only)
• IDFPR - license
• OIG - government healthcare programs: sanctions, restrictions or exclusions from participation
• NPDB - medical malpractice

To approve the recommendation of the Board of Health Credentialing Committee related to the appointment of Jennifer Gassman, APRN, and the reappointment of Sana Ahmed, M.D., Aileen Benjakul, M.D., Joanna Caban, M.D., Ashok Nagella, M.D., Anatoliy Pyslar, M.D., Hargurmukh Singh, M.D., and Toni Steres, APRN.
