FY 2020 AIDS Foundation of Chicago Ryan White Part B, $59,042 - Grant Approval - Zun

On July 15, 2020, the AIDS Foundation of Chicago (AFC) awarded LCHD/CHC a total of $59,042 to perform activities and obligations to employ an additional 1.0 FTE medical case manager for assessments, care plans, counseling, risk reduction counseling and referrals of HIV+ and self-sufficient HIV+ individuals. Outpatient ambulatory care, and medical nutrition therapy will also be provided to unduplicated HIV-infected individuals residing in the Collar Counties area. LCHD/CHC use of these funds are aligned to support medical case managers’ salaries and fringe benefits. The grant period is from April 1, 2020 - March 31, 2021.
