As defined in the Governing Council bylaws, one of the key responsibilities assigned to the Council is the approval of hours of services delivered at Lake County Health Department and Community Health Center (LCHD/CHC) Federally Qualified Health Center (FQHC) locations. LCHD/CHC is bringing the attached revised hours of operation for the FQHC sites listed for review and approval.

To approve the change in hours of operation for the Lake County Health Department and Community Health Center Federally Qualified Health Center (FQHC) locations as presented.
