• There is a need to contract with an Occupational Health provider to complete employee vaccinations, pre-employment physicals, and fit testing to comply with Occupational Safety and Health Administration (OSHA) and grant regulations.
• A Request for Proposals (RFP) was issued and extended to 11 qualified vendors and sealed proposals were received from two local venders.
• An interdepartmental evaluation committee comprised of the Human Resources, Division of Transportation, and Health Department interviewed two vendors and based on the evaluation criteria in the RFP identified Visa Health Systems to be the most qualified vendor who best meets the needs of the County.

RESOLUTION
WHEREAS, in accordance with Section 5-102 of the Lake County Purchasing Ordinance, proposals were called for by publication for occupational health services for Lake County; and
WHEREAS, subsequent to said publication, request for proposals were extended to 11 vendors; and
WHEREAS, sealed proposals were received from 2 vendors and opened in the County Building, Waukegan, Illinois, on June 2, 2020, at 11:00 a.m.; and
WHEREAS, based on the criteria set forth in the Request for Proposals, an ad hoc committee selected Vista Health Systems, Waukegan, IL, to be in the best interest of Lake County.
NOW, THEREFORE, BE IT RESOLVED, by this County Board of Lake County, Illinois, that the Purchasing Agent be authorized to enter into the contract with Vista Health Systems to complete vaccination, Personal Protection Equipment (PPE) fit testing, and pre-employment physicals for County employees in an estimated amount of $180,000 per unit cost pricing out of account number 206-1440010-71420-000-000-000-00000.
