Clinical Risk and Patient Safety Management (CRPSM) Program and Training Plan - Koppit

The Lake County Health Department Clinical Risk and Patient Safety Management (CRPSM) Plan supports the expectation that all staff are to:
• be knowledgeable about and participate in risk management activities,
• assist with the implementation of recommended improvements, and
• identify risk events and opportunities for improvement.
This Plan describes the support for the establishment of a culture within the Lake County Health Department and Community Health Center (LCHD/CHC) that emphasizes:
• implementing evidence-based best practices
• learning from error analysis
• providing constructive feedback and learning opportunities rather than blame and punishment
• understanding that incident reporting will improve the system and prevent future occurrences
The annual Risk Management Report of risk assessment for 2019 was provided to the Council at their February 11, 2020 meeting. The Governing Council is also required to annually review the LCHD/CHC programmatic goals and objectives set within the CRPSM Program and Training Plan and established based on the risk assessment information provided within the annual report. The overall goal of the CRPSM is to reduce system-related errors and potentially unsafe conditions by implementing continuous improvement strategies to support an organizational culture of safety. The CRPSM Program and associated Training Plan for 2020 are provided for Council review and approval.
