Presented is credentialing information on three providers. The Medical Director has verified the privileges requested are appropriate for the provider’s training and experience.
The Medical Director has studied the applications, raised questions where necessary, and based upon satisfactory responses from the applicant, recommends approval of the request for privileges.

• Heather Martelon - Full-time Family Practice APRN, started March 16, 2020 at North Chicago Health Center.
• Akilah Yates - Full-time Licensed Clinical Professional Counselor (LCPC), started March 2, 2020 at Belvidere Medical Clinic.
