Clinical Risk and Patient Safety Management - Koppit

Annually, the Lake County Community Health Center Governing Council is required to review responses to the programmatic goals and objectives set within the Clinical Risk and Patient Safety Management Plan (CRPSM).
The CRPSM Plan was established to reduce system-related errors and potentially unsafe conditions by implementing continuous improvement strategies in support of an organizational culture of safety. The program addresses clinical operational and patient safety risks that supports the expectation that all staff are:
• knowledgeable about and participate in risk management activities;
• assist with the implementation of recommended improvements; and
• identify risk events and opportunities for improvement.
The CRPSM Plan also delineates the support for the establishment of a culture within the Lake County Health Department and Community Health Center (LCHD/CHC) that emphasizes:
• implementing evidence-based best practices,
• learning from error analysis,
• providing constructive feedback and learning opportunities rather than blame and punishment, and
• understanding that incident reporting will improve the system and prevent future occurrences.
The attached report provides specific information regarding actions taken during 2019.
