• The County Board, at its June 14, 2016 meeting, approved an appropriation of $355,000 of ¼% Sales Tax for Transportation funds and a consultant agreement with Bollinger, Lach and Associates, Inc., Itasca, Illinois, at a cost of $292,165.67.
• Change Order Number One is needed for additional Phase II engineering services that include land acquisition, plat of highway revisions, and drainage design for updated Bulletin 70 rainfall data.
• This project is included in the highway improvement program with target construction in 2022, and designated as Section 15-00118-12-CH.

R E S O L U T I O N
WHEREAS, Miller Road, County Highway 2, is a designated route on the County highway system; and
WHEREAS, it would be beneficial to the safety and welfare of the traveling public if Miller Road, at US Route 12, could be improved by adding turn lanes and modifying the traffic signal; and
WHEREAS, the County Board, at its meeting of June 14, 2016, approved an appropriation of $355,000 of ¼% Sales Tax for Transportation funds for Phase II design engineering services for the intersection improvement of Miller Road at US Route 12; and
WHEREAS, this project is currently under contract with Bollinger, Lach and Associates, Inc., Itasca, Illinois, in the amount of $292,165.67; and
WHEREAS, Change Order Number One, in the amount of $77,682, is needed for additional Phase II engineering services to the original contract; and
WHEREAS, a supplemental appropriation of funds is necessary for the additional scope of work; and
WHEREAS, the Lake County Purchasing Ordinance requires the Public Works, Planning and Transportation Committee and the Finance and Administrative Committee to approve change orders with cumulative totals above 10 percent of the original contract.
NOW, THEREFORE BE IT RESOLVED by this Lake County Board, that the County Engineer of Lake County is authorized and directed to execute on behalf of Lake County, Change Order Number One in the amount of $77,682 for additional Phase II engineering services for the intersection improvement of Miller Road at US Route 12.
BE IT FURTHER RESOLVED that there is appropriated a supplemental amount of $55,000 of ¼% Sales Tax for Transportation funds for these additional Phase II engineering services, and designated as Section 15-00118-12-CH.
