The LCHD has received $100,000 in funding from HFNLC to support medical nutrition therapy for patients with uncontrolled diabetes. The grant will fund 1.6 FTE dietitians including one new staff and .6 FTE of an existing staff member. The grant period is November 25, 2019 - November 24, 2020. The grant will also provide funding for food supplies (healthy snacks) which are distributed during diabetes-related health education. As the grant includes funding for an existing staff member, the attached expense budget is less than the full grant amount.
Suggested Motion
To approve the grant funding of $100,000 from the Healthcare Foundation of Northern Lake County to support dietitian services for LCHD patients with diabetes.
