The Lake County Health Department and Community Health Center (LCHD) has developed a QMS Guide that establishes a systematic and uniform approach to quality improvement throughout the agency. The guide applies to all staff and describes our quality improvement framework, principles, procedures, and roles and responsibilities of employees. The guide is in accordance with The Joint Commission, Health Resources and Services Administration (HRSA), and Public Health Accreditation Board (PHAB) requirements.
