The Gorter Family Foundation has awarded the Lake County Health Department and Community Health Center (LCHD/CHC) a grant in the amount of $40,000 for the period of July 1, 2019 - June 30, 2020. These funds will be used to support a coordinated intake program in North Chicago. This program will support a staff member who will serve as a single intake specialist for North Chicago children and families in need of home visit and supportive services.
