Presented is credentialing information on four providers. The Medical Director has verified the privileges requested are appropriate for the provider’s training and experience.
The Medical Director has studied the application, raised questions where necessary, and based upon satisfactory responses from the applicant, recommends approval of the request for privileges.
Amy Holmgren - Full-time Psychiatric Advanced Practice Nurse (Grand Avenue Health Center - CABS Program)
Christina Harmon - Full-time Licensed Clinical Social Worker (Zion Health Center)
Dr. Susan Felsenthal - Flex Internal Medicine Physician (started June 24, 2019; will primarily cover our Addictions Treatment Program/Medication Assisted Treatment (ATP/MAT) and Women’s Residential Services (WRS) program with the potential to fill in at other LCHD/CHC sites)
Dr. Jackie Rajani - Psychiatry Resident from Rosalind Franklin University (Grand Avenue Health Center)
