We are recommending the appointment of the individuals noted in the motion below. For the individuals recommended for appointment, we have performed the following verifications and there are no identified issues:

• Sex Offender registry check (CABS program providers only)
• IDFPR - license and controlled substance license
• OIG - gov’t healthcare programs: sanctions, restrictions or exclusions from participation
• AMA - medical school and residency
• ABMS - board certification
• NPDB - medical malpractice
• Hospital Privileges

To approve the recommendation of the Board of Health Credentialing Committee related to the appointment of Leslie Zun, M.D., and James Jupa, M.D.
