Clinical Risk and Patient Safety Management - Koppit

As a Federally Qualified Health Center (FQHC), the Lake County Health Department and Community Health Center (LCHD/CHC) is eligible for lawsuit coverage under the Federal Torts Claims Act (FTCA). As part of the terms of this coverage agreement, the FQHC is expected to have and manage an active clinical risk management program. At the end of last year, the Governing Council approved the Clinical Risk and Patient Safety Management (CRPSM) Plan that supports the expectation that all staff are:
• to be knowledgeable about and participate in risk management activities;
• to assist with the implementation of recommended improvements; and
• to identify risk events and opportunities for improvement.
Additionally, the approved CRPSM Plan delineated the support for the establishment of a culture within the LCHD/CHC that emphasizes:
• implementing evidence-based best practices;
• learning from error analysis;
• providing constructive feedback and learning opportunities rather than blame and punishment; and
• understanding that incident reporting will improve the system and prevent future occurrences.
Annually, the Governing Council is required to review the LCHD/CHC response to the programmatic goals and objectives set within the CRPSM Plan. These goals seek to reduce system-related errors and potentially unsafe conditions by implementing continuous improvement strategies to support an organizational culture of safety. The attached report and CRPSM training plan provide specific information regarding actions that were taken this past year.
