United Way Success by Six Program Grant - Kritz

The Lake County Health Department and Community Health Center (LCHD/CHC) received renewal funding from the United Way of Lake County for the Success by Six Program. The grant period is January 1, 2019 - December 31, 2019. The total grant amount is $30,000, however, $25,000 was already budgeted in County FY19 so this appropriation is for $5,000. Appropriation is unbalanced as grant funds will be used to offset existing salary and fringe dollars for staff. The grant supports home visits for families in the communities of Waukegan, Zion and North Chicago.

To approve $5,000 in grant funding from the United Way of Lake County to support the Success by Six program.
