• Lake County’s Facilities and Construction Services (FCS) Department identified a need to enter into a professional services agreement to provide architectural and engineering services for the Belvidere Medical Building (BMB) Chiller Replacement and Annex Fan Coil Replacement in February 2018.
• Kluber Inc. was awarded the initial contract in the amount of $49,800 in accordance with the Local Government Professional Services Selection Act (50 ILCS 510/1 et seq) and a prior satisfactory relationship.
• FCS has identified a need for Contract Modification Number One to increase the scope of work to include additional fan coil replacements and a heat exchanger unit at the Annex and additional fan coil replacements at BMB in the amount of $83,975, increasing the existing contract to $133,775.
• The additional scope of work includes additional hours needed to complete the construction document phase to include additional scope of work, bidding and negotiation phase and construction administration phase.

R E S O L U T I O N
WHEREAS, Lake County Facilities and Construction Services (FCS) identified a need to enter into a professional services agreement to provide architectural and engineering services for the Belvidere Medical Building (BMB) Chiller Replacement and Annex Fan Coil Replacement in February 2018; and
WHEREAS, Kluber Engineers and Architects (Kluber Inc.) was awarded the initial contract in the amount of $49,800 in accordance with the Local Government Professional Services Selection Act (50 ILCS 510/1 et seq) and a prior satisfactory relationship; and
WHEREAS, FCS has identified a need for Contract Modification Number One to increase the scope of work of the project to include additional fan coil replacements at BMB, additional fan coil replacements at the Annex, and a heat exchanger at the Annex in the amount of $83,975, increasing the existing contract to $133,775; and
WHEREAS, the additional scope of work includes additional hours needed to complete the construction document phase to include additional scope of work, bidding and negotiation phase and construction administration phase.; and
WHEREAS, Lake County, has selected Kluber, Inc., in accordance with the Local Government Professional Services Selection Act, 50 ILCS 510/1 et. seq.
NOW, THEREFORE BE IT RESOLVED by this Lake County Board ratifies the existing contract between Lake County and Kluber, Inc. of Gurnee, Illinois in the amount of $49,800 the Purchasing Agent is authorized and directed to execute on behalf of Lake County, Contract Modification One in the amount of $83,725 for architecture and engineering services for the Belvidere Medical Building Chiller Replacement and Annex Fan Coil Replacement
