SAMHSA has provided the LCHD/CHC with $410,000 in funding through the Grants to Benefit Homeless Individuals (GBHI). These funds are for an initial budget period of September 30, 2018 through September 29, 2019. The overall project period is 5 years with $400,000 anticipated in each of the subsequent years for a total amount of $2,010,000. 

 

Grant funds will be used to create a mental illness/substance abuse team that will serve individuals who have a substance abuse/co-occurring disorder and are experiencing homelessness. Counseling and case management will be delivered by LCHD/CHC staff working outside of the Federally Qualified Health Center (FQHC). Psychiatry will be delivered inside the FQHC. As psychiatry is included, the grant requires approval by the Governing Council as well as the Board of Health. A total of .1 FTE psychiatry time (4 hours a week) will be added through this grant. This equates to a total amount of $18,798 that will be appropriated into the FQHC. The balance of the grant award will be managed outside of the FQHC. 

 

As part of this grant, LCHD/CHC will have a vendor agreement with PADS Lake County to provide outreach services. Once identified, PADS staff will connect clients to LCHD/CHC providers and staff for psychiatry, counseling and case management. 

 

Suggested Motion

To approve $410,000 in grant funding from the Substance Abuse and Mental Health Services Administration to form a mental illness/substance abuse team that will serve individuals experiencing homelessness in Lake County.
