Dental Laboratory Services Request For Proposals #18168 - Nordstrom

The Lake County Health Department and Community Health Center (LCHD/CHC) has a need to contract with suppliers for dental laboratory services for six LCHD/CHC dental sites for removable prosthodontics (dentures and partials). All work is fabricated in the United States and all materials used are FDA approved and ISO certified.
A legal notice was published in the Lake County News Sun and the Request for Proposals (RFP) was posted to the Lake County Purchasing website.
Sealed proposals were received from two suppliers. The evaluation committee reviewed the RFP submittals and found the two suppliers possessed the experience and capabilities to perform the requirements and specifications outlined in the RFP and is requesting the authorization to contract with the following suppliers:
• Popp Dental Laboratory, Greendale, Wisconsin
• D&S Dental Laboratory, Waunakee, Wisconsin
Suggested Motion
To approve the award of a contract for dental laboratory services to Popp Dental Laboratory, Greendale, Wisconsin, and D&S Dental Laboratory, Waunakee, Wisconsin and to authorize the Executive Director or designee to execute the necessary documents with funds budgeted.
