Staff Summary
• On May 7, 2018 a purchase order for $225,000 was issued to Fab Tech Waste Water Solutions, LLC for rehabilitation to extend the service life of the clarifiers.
• Two change orders were issued to address unforeseen conditions in clarifier number one. The work included fabricating a new stainless steel suction header, flushing device and cage repairs.
• Change Order Number Three replaces the influent wells for both clarifiers and fabricates a stainless-steel suction header and repairs the cage of clarifier number two. This equipment was originally intended to be rehabilitated with sand blasting and recoating but the amount of corrosion was found to be extensive, requiring the components to be replaced.
• In accordance with Article 8, section 101(2) of the Lake County Purchasing Ordinance, these change orders were not reasonably foreseeable at the time the contract was signed.
• Change Order Three is in the amount of $37,375 increasing the previous contract amount from $242,775 to $280,150.
