Staff Summary

                     On July 28, 2017 a purchase order was issued to Donohue and Associates, Inc., in the amount of $130,750, for the preparation of design and bidding documents for Vernon Hills Water System Electrical and Mechanical Upgrade Project

                     The County issued a Statement of Interest (SOI) and consultant services were procured in accordance with the Purchasing Ordinance and the Local Government Professional Services Selection Act.

                     Change Order One, in the amount of $34,515, was approved on May 2, 2018 and addressed unforeseen increases in the scope of design work at the eight individual sites.

                     Change Order Number Two increases the scope of the design for two of the eight sites.  An existing well house at Hawthorn Tower was originally planned to be repurposed to house electrical panels but a less expensive location was found and the well house is now designated for demolition.  The pumping equipment at the Grosse Point facility is being added to the scope.  This work was originally part a new reservoir project but the reservoir has since been relocated.

                     In accordance with Article 8, section 101(2) of the Lake County Purchasing Ordinance, this change order was not reasonably foreseeable at the time the contract was signed.

                     Change Order Number Two is in the amount of $9,450 increasing the previous contract amount from $165,265 to $174,715.
