Staff Summary
* The Lake County Sheriff's Office (LCSO) has been awarded a grant from the Healthcare Foundation of Northern Lake County for continuation of the Jail High Utilizer Diversion and Health Engagement Management Project in the amount of $100,000.
* The LCSO will work to reduce the re-incarceration of persons who have been identified as jail high utilizers with a focus on those who are in protected classes and/or from low-income communities.
* The High Utilizer Diversion and Health Engagement Program will continue to provide intensive case management services to the communities listed above and use that case management to reduce recidivism.
* The expected grant outcomes of this program associated with the intensive case management with the high utilizers will remain the same and is considered a stop gap measure while a sustainable funding source is sought.
* The creation of this program requires the LCSO to enter into an agreement with NICASA to provide intensive case management services that meet grant requirements.
* NICASA will be required to provide and meet performance measurement and reporting standards.
* The agreement shall be for an initial one-year period.
* The County and the LCSO reserves the right to extend the agreement for two additional one-year periods, subject to the acceptable performance of NICASA.
* These grant funds will support the continuation of the Jail High Utilizer Diversion Health Engagement Project through May 20, 2019.
